Functional Neurological Disorder (FND)
Functional neurological disorders, also often referred to as ‘psychogenic movement disorders’, ‘dissociative movement disorders’, ‘somatoform disorders’ or ‘conversion disorders’, are a group of neurological disorders in which symptoms such as paralysis, coordination disorders or speech problems appear to be neurological, but no organic cause can be identified. These disorders are often (but not always) associated with stress, trauma or other psychological stress and can manifest themselves in a variety of ways, for example as motor or sensory impairments.

Despite its significant impact on the quality of life of those affected, FND is unfortunately still a frequently under-diagnosed and under-treated condition. Many patients often do not receive adequate treatment for a long time, as FND is frequently misunderstood or wrongly dismissed as a psychosomatic complaint. However, early diagnosis and specially tailored treatment are crucial in helping those affected to regain their quality of life and regain control over their body.
How Can Physiotherapy Help
Physiotherapy offers a valuable approach to the treatment of FND by helping affected individuals to restore their motor function and regain control of their body. As FND is often associated with motor and sensory symptoms such as paralysis, incoordination or involuntary movements, physiotherapy treatment aims to alleviate these symptoms and improve neurological function.
A personalised therapy plan can help to:
- Train movement perception
Physiotherapists use targeted exercises to promote body awareness and coordination of movement . This is particularly important as the perception of one's own body is often impaired with FND.
- Promote neuroplasticity
Specific movement exercises aimed at reactivating certain areas of the brain can stimulate neuroplasticity - the brain's ability to adapt to new demands and partially restore lost functions.
- Regulate muscle tension
Many patients with FND suffer from uncontrollable muscle tension or paralysis. Physiotherapists use relaxation techniques and targeted mobilisation to stabilise the muscles and improve freedom of movement.
- Relieve pain
Using manual techniques, functional movement exercises and other therapeutic interventions, physiotherapy treatment can also alleviate pain caused by poor posture or muscle tension.
- Integrate psychosomatic aspects
As FND is often associated with psychological stress or traumatic experiences, physiotherapy helps to bring body and mind back into harmony. Therapeutic dialogue and an understanding of psychosocial factors play an important role in the healing process.
Physiotherapy for FND takes a holistic approach that not only treats the physical symptoms, but also helps to improve the quality of life and self-confidence of those affected. The aim is to promote independence through regular and targeted therapy and to help patients achieve greater independence and enjoyment of life in the long term.
This requires close multidisciplinary collaboration between various specialists, such as neurology, psychology, occupational therapy and, if necessary, speech therapy and psychotherapy in order to address the various aspects of the disorder holistically and enable patients to achieve a comprehensive and sustainable improvement in their quality of life.
